*********If you receive a "user name you have chosen is already in use". Please call GC Health Department at 856-218-4102 so we can confirm your information needed to register.

*********If you receive a "we have no record of you in our database" click ok then click create new account.

Once registered please fill out a consent form for desired services under the forms tab in patient portal and click submit.

*Please note: Only one patient portal account can be created per email address and a completed influenza or covid consent is only valid for 60 days.
